* D.N.I.
* Dirección de correo electrónico del paciente
* Numero de teléfono del paciente
* Fecha de nacimiento y domicilio
Se deberá adjuntar el resultado en formato PDF, el Colegio lo enviará a Dirección de Laboratorio.
